DIFFICULT DRY STICKY EAR WAX REMOVAL – EP 83


hi everybody this is Rhys barber from
audiology associates thank you very much for watching our video today we have
another bilateral ear wax eaxtraction video for you so you can see this gentleman’s
got some quite sticky ear wax not completely blocking the ear canal but just causing
a few issues with with hearing using the Jobson horn you can see a little bit of
ear wax on the jobson I’ve used it just to probe the ear wax first roll just to see how sticky this ear wax is the reason we’re opting for the Jobson horn to remove
this ear wax rather than the zoellner tube is because this the ear wax is so sticky in
nature when you use a zoellner tube it just blocks the end of the zoellner very
quickly so what you have to do is keep clearing the zoellner and a lot and you get
a lot more out using the Jobson horn so what I like to do is try and take the
central portion of ear wax away first of all so create this little Valley in the
middle and then use the zoellner tube to pull the ear wax away from the ear canal walls
you just blocked up very quickly there into the center just folding the ear wax in
on itself because to take the ear wax away using the the Jobson horn along the
ear canal wall is quite uncomfortable it it feels like a scraping sensation I’m out
pinching sensation so that’s why I got them to use the suction to pull the ear wax
away from the ear canal walls as it’s more comfortable for the patient you can see
just trying to fold it all into the center of the ear wax also trying to loosen
sometimes when you get a grip on on the sort of base section of ear wax even lift it
it will detach quite a bit of it off the ear canal wall as well so rather than trying
to pull it from the back forwards which can then stick it to the ear canal walls
you’re going down which we can lift it off and then take it are going to be a
piece put some olive oil into the ear canal there this just reduces the stickiness
of the ear wax it reduces the adhesion to the ear canal wall so it does help us to
some things get this out in one piece you can see they’re just lifting
all the ear wax are wait and see it coming away and slightly bigger pieces now the
area’s just starting a little bit of moving you can see we’ve unstuck quite a
lot of this one the ear canal wall using the suction of the zoellner allowed to maneuver
this block towards the entrance to the ear canal doesn’t really want to come
out just yet so we’re going to go see we can get behind it with the the Jobson
now just sliding trying to roll you can see this back section ear wax a lot harder
than the front section so we’re able to roll this you can see little bits of
here and they wish you see on quite a few of the videos from ear trims you
know yeah is that block so you can see it’s quite dark and colour quite a large
piece and if we take a look then the others the ear drum it’s a little bit of
redness around the ear canal there we’ve been maneuvering this down a very
similar story on the other side we can see we’ve got the gap at the top of the
ear wax there the front end looks quite shiny so it’s almost difficult to tell
whether that’s hardened up or sticky ear wax so we’re just gonna use some suction
yeah it looks a little bit harder than the than the other side so doing the
same process again just trying to lift from the base of the ear canal but it
doesn’t look like it’s gonna budge so we’re gonna use the Jobson now just to
get behind this or you can see is the base of the ear canal basically the ear wax
nearest the ear canal walls is quite hardened up whereas the top is is very
soft and sticky so we’re just gonna use the same process you can see the groove
there that the jobson horn dug into that that ear wax there so we’re just gonna
try a name for the gap above the ear wax not wanting to go in too deep you can see how sticky that ear wax is it’s really
sticking to the ear canals were trying to maneuver it out so we can use the same
process again they’re just lifting off the ear canal wall they’re going just see it
starting to move okay went back in with the Jobson home
so it’s not quite budging at this sticky type of ear wax can be quite frustrating to
get out because sometimes it will come out as we saw on the previous ear in a
nice large lump sometimes it starts to break down as we get in this we’ve gotta
keep switching between tools then just to use those on rather than to remove
the ear wax using it to prepare the ear wax for the jobson horn to go in and and pull
that down you can see we’ve got a slightly harder lump of ear wax at that at
the base now so just put a little bit of olive oil in same reasons before just to
reduce some adhesions you can see it’s still stuck to the ear canal walls there and
just to help to slide this ear wax out so if you imagine the all of our forms it
almost like a lubrication around the edge of the ear canal as well so not
only does it reduce the stickiness but it helps us to slide that ear wax across the
skin can see you’ve got an end of ear wax here just protruding from the ear canal just
try to wrestle this out broke a little piece off the end
then use the jobs and hard to get behind this block now if we can you can see
it’s just difficult to get behind it it’s just pulling it forward slightly
and I just under roll now here we go didn’t want let me pick it up so we just
use the suction to get those pieces away and there’s the eardrum looking nice and
healthy so this is what we removed you can see the first section here the
first piece on the left hand side is the larger bit we took out the first ear so
you can see all the bits and pieces there you’ve got the jobs in horn stand
essays are on the tube we can see some of the bits that got sucked into the
suction to suction pot as well as we were going well thank you again for
